This notice explains what happens to the details you send through the “Request a conversation” form on ancillaryrevenue.health.
What we collect
The details you enter in the form: your name, work email, company or practice name, role and state, plus your note if you add one. We also record that you agreed to be contacted about your request.
The form doesn’t ask for health information or other sensitive personal information. Please don’t include patient information or other sensitive personal details in your note.
Why we collect it
To reply to your request and to arrange a first conversation.
What happens when you send the form
Your submission is sent by email so that we can read it and reply. For now, request emails are delivered to a mailbox on the kaicare.ai domain, which belongs to KaiCare.AI, until Ancillary Revenue has a mailbox of its own. We’ll update this notice when that changes.
We don’t keep form submissions in a separate database, and the service that sends the email doesn’t log what you entered.
Like any website, this one is delivered by hosting providers that process technical information, such as your IP address and browser type, to deliver pages and to protect the form from abuse.
Access, correction and deletion
To ask what we hold about you, or to ask us to correct or delete it, reply to any email we have sent you, or use the form on our home page and say in your note that it’s a privacy request. Tell us what you’d like us to do. We’ll reply by email, and we may ask you to confirm that the request came from you.
Changes to this notice
If we change this notice, we’ll update the date at the top of this page.
